AdventHealth Carrollwood Hospital in Tampa, Florida has selected HealthSearch Partners to conduct a search for its next Director of Care Management. The 103-bed non-profit healthcare facility serves patients in Hillsborough County and the north Tampa area. The hospital is part of the AdventHealth West Florida Division that encompasses more than 2,300 licensed beds in 12 hospitals and one long-term acute care hospital with more than 15,000 staff members.
The Director of Care Management will lead the overall operations of the hospital’s Care Management Department that includes Coordination of Care, Centers for Medicare and Medicaid Services (CMS) Conditions of Participation, Discharge Planning, Social Services, Readmission Prevention, Length of Stay, Excess Days, Avoidable Days, Value Based Care partnerships, Care Managers’ roles in Multidisciplinary Rounds and Care Transition.
The ideal candidate will have a solid track record and a minimum of three to five years of experience in leading Case Management or Care Management in an acute care setting. Nursing applicants will preferably have a Master of Science (MSN) in Nursing degree. Social workers will preferably have a Master of Social Work (MSW) degree and be a Licensed Clinical Social Worker (LCSW). All applicants must have the ability to be licensed in Florida.
